Concierge Case Management

My experience working in the field in various levels of care has provided me with insight into how broken the behavioral health system can be. There are many great providers, treatment programs, and services available; however, there is little continuity of care when a person moves locations or programs.

Concierge case management was born out of the need for families to have a 'point person' to follow their loved one throughout various systems of care and ensure that all parties were in communication with each other about the next steps in treatment. This service is meant to be an all inclusive service that bridges the gap between transitions and follows the person despite location, level of care, or current presenting problem.

The ideal client for this service may:

Have difficulty engaging in 'traditional' systems of care i.e. therapy or groups
Have been to multiple treatment programs with mild or moderate success
Have not been able to sustain their recovery after discharge from treatment
Have limited social or recovery support networks
Have a mental health and/or substance use disorder that prevents them from maintaining housing, relationships, and/or employment
Require in-home or community support instead of going to an office to see their providers

Due to the intensive needs of clients who require this level of service, I have limited availability for case management clients.
